Making major healthcare decisions should be made with a knowledgeable, professional advocate that is on the patients side. This is rarely the case when the hospitals' doctors and discharge planners are in control. These hospital staff will take whatever is the quickest and easiest route for themselves. Usually, their options are nursing homes and long-term care facilities. However, if you ask any senior where they would prefer to live, it will be in their homes, the place where they are comfortable and have some control of their care.

Call us before the discharge planning in the hospital begins and we will meet with our patient, his/her family and their physicians to make sure your loved ones desires and needs are met. We are here to advocate for our patient and to make their living arrangements a top priority.

Once our patient is in their home, Park Cities Healthcare RN case managers make weekly visits, or more frequently as needed. These visits include physical assessment and medication management. We will be the heart of care coordination between our patient, his/her caregivers and the rest of their healthcare team.
