At Park Cities Healthcare Consultants,LLC we provide a high level of comprehensive consultation/assessment/coordination of care which allows for our clients to make one call and know that every detail will be handled. Our RN case manager will provide detailed explanation of services offered and then start the process of getting our patient home, or if already home, putting in place all of the detailed care that allows them to be safe and as highly functional as possible.
An Initial Consultation includes: physical assessment of patient, safety assessment of their home, retrieval of records from all treating healthcare providers, medication coordination and set up, as well as placement of caregivers in the home should this be a requirement for safety. Set up will incorporate a delivering pharmacy, if requested. At this time we will initiate Long Term Care Insurance policies if our patients family needs assistance. Fee: $500.00/includes a Plan of Care
Care Management: This service includes physical assessment and medication management on a weekly basis. Every aspect of our patients care will be addressed by a RN at this time and coordination of care with healthcare provider and family will be provided. Fee: $500.00/month after initial consultation
Concierge Services include attending appointments with our patient or additional home visits requested by our patient or his/her family and will be billed at $125.00/hour
Park Cities Healthcare Consultants,LLC is a Fee for Service healthcare provider and as such is covered under most Long Term Care Insurance policies, and we will gladly bill them directly. For your convenience we also accept most credit cards.
